HPV Vaccination Information
Dear Parent/Guardian,
The HPV vaccine has been delivered in the UK for over 15 years. This vaccine is offered to both girls and boys in year 8. The Vaccine helps protect against the human papilloma virus. This virus increases the risk of developing some cancers later in life such as cervical, mouth, throat, anus and genitals. The HPV vaccine is now given as one dose.
You will have all received information or links to information about this vaccine, we encourage that this is discussed with your son/daughter, then complete the consent form.
Any person with parental responsibility can consent for a child to be vaccinated. Any child under the age of 16 years old can self consent for vaccination if assessed as competent by a qualified nurse. This is known as being Gillick Competent.
This will only be done if the nurse is confident that the child understands all the information given, and only then can they consent for the vaccination themselves.

You can also talk to the Nurse at your child’s school, your GP or Practice Nurse.
